New Patient appointments can vary in length from 30 minutes to 3 hours, depending on what type of allergy testing may be needed. This time will allow our doctors to gather a detailed allergy history, perform comprehensive allergy testing, and to discuss a plan of treatment that's right for you!.
Please arrive to your appointment 15 minutes early. To save time, please fill out New Patient paperwork in advance and email it to lsa@lakeshoreallergypc.com. You should bring your insurance card and driver’s license with you.
Patients under the age of 18 require a parent or legal guardian to be present for the visit.
If you are scheduled for allergy testing, please discontinue any antihistamines (found in allergy medicines or cough syrups) for 3 days prior to your testing.

We do not currently cover inpatient services including desensitizations or inpatient consults.
However, we are affiliated with several hospitals in order to expand access to specialist care in our community. Our hospital affiliations include:

We gladly accept self-referred patients, but your insurance may require a referral in order for services to be covered. Please check with your insurance plan.
If you are a self-referred patient, we request that you allow your primary healthcare provider to send documents pertaining to other health conditions you may have so we can best serve you.

FAQs

A link to our patient portal, FollowMyHealth, is provided at the top of our website. Our portal will allow you easy access to your clinic notes, medication information, and appointment information.
We are currently working to update and expand our portal services to allow you to pay your bill, submit refill requests, ask billing questions, and schedule shot appointments. Our goal is to have these services available by Fall 2025.
We do not currently allow for patient messages to be sent via portal. We request that you call our nurse advice line and leave a message. Our nurses will return your call within 24-48 business hours.

All requests for school forms must be submitted via email at lsa@lakeshoreallergypc.com. Hard copies may also be dropped of in person. Please submit school-specific forms or FMLA forms at the time of your request.
There is a $10 charge for school forms that must be paid before the forms will be processed. We require a visit within the last year to fill out forms.
School notes or return-to-work notes will only be provided on the day of your appointment.

You may request a prescription refill by asking your pharmacist to send us a request, or you may call our nurse line. Please leave the patient’s name, date of birth, medicine being requested, and a contact number. You must have been seen within the previous 12 months to have a prescription refilled.
Prescription refill requests will only be processed during normal business hours and should not be submitted via the after hours phone number.
Please allow up to 2 business days for refill requests.

We often have same day sick visits available. Please call our office to schedule an appointment.

Statements are processed in the first week of every month and will be mailed to your current address on file. Our system is undergoing updates so that statements will soon be sent via text message and available through our patient portal.
Delinquent accounts will be sent to RNP Collection Agency.
Starting in Spring 2025 we will begin requiring a credit card on file so that we can easily process your payments. No charges will be made to the card without your prior consent.
If you require a payment plan or have additional questions about your bill, please contact our office by calling our office and selecting the prompt for the Billing Department.
